At H.E.R. T.H.O Hearts LLC, we believe every individual deserves a care plan that reflects their voice, respects their choices, and supports their unique journey toward independence. Our Personalized Care Plans are custom-designed in collaboration with each client, their circle of support, and regional center coordinators to ensure services are tailored to individual goals, needs, and daily living preferences. Each plan includes: A full assessment of strengths, support needs, and personal goals Ongoing input from the client, family members, and advocates Clearly defined outcomes in housing stability, health, community access, and personal development A dedicated support team to monitor progress, adapt services, and advocate as needs change Our plans are not just documents — they are living roadmaps designed to evolve as our clients grow, achieve milestones, and define what independence means to them.
